Follow Your Heart — Susanna Tamaro

Fearing her encroaching death, an elderly woman decides to write a long letter to her granddaughter, in the shape of a diary. Part love-letter and part confession, it is a bequest of advice from an old woman who acknowledges that she has always bowed to convention and repressed her feelings.
